My guess is that they are indeed using something different to cause widespread reality blindness, between it not affecting Joshua (since the Cure hasn’t historically worked on anyone besides Mads) and Mr. Green expressing a bit of uncertainty about whether the gas would work even on a mad genius, which suggests that it’s a new formulation.
IIRC, the Cure causes reality blindness as a side effect of neutralising Mad Science Disorder (aka Walton’s Disorder in-universe); if you don’t have that particular strain of madness – which makes you “sane” at least in a colloquial sense, no matter how strange or psychologically unwell you might be in other ways, or how much affinity you might have for advanced yet unethical scientific practices in Dr. Lee’s case – then it doesn’t do much of anything. And it doesn’t look like Anasigma’s managed to change that.

Being Mad tends to make one insane, because there’s so much going on in their head that they can’t think like a normal human being. But being insane doesn’t cause one to go Mad.
I’m thinking that the reason for Sundance’s prediction is that, if you don’t shoot the men with beards far enough away, one of the other dogs may try to catch them and bring them back.
